My Goals

HEALTHY FOODS CENTER PILOT PROGRAM

While working as a Community Outreach Coordinator for a local pediatric practice comprised of 11,000 patients, I became the team lead for the clinic’s Fight Against Childhood Hunger Initiative. Simultaneously, I utilized my skills in graphic design, strategic planning and website development. 

As part of this initiative, I joined the Health Foods Center (HFC) community partners team. This team was made up of representatives from 6 local hospitals/clinics and Roadrunner Food Bank’s (RRFB) health and wellness team. The HFC is a pilot program initiated by RRFB. Partnering clinics are able to refer patients to the HFC based on an identified medical condition and food insecurity.

Collectively, this team of partners sought to identify and address socio-economic barriers patients may experience when trying to access healthy foods. Collaboratively we developed programming to demonstrate a direct correlation between nutrition and successful chronic disease prevention and management. With the desire to grow patient resources, Kids Cook, Fūdrr and Prescription Trails became extended partners.

My Goals Strategic Communications Plan and Design Elements

Seeking to improve the health outcomes of patients, more effective tools were being sought to facilitate a full circle conversation between patients, providers, the HFC’s nutrition education team and other resources. In response to this need, I developed the My Goals Strategic Communications Plan.

Simultaneously, both Prescription Trails and the developers of Fūdrr a healthy lifestyle mobile ap became two new key partners to help patients engage with educational tools, track their progress in measurable and concise way to report back to their doctors.

Tool Development

I designed a custom prescription pad for physicians to write out specific, measurable goals related to a patient's nutrition and exercise needs.

Icon Development

I developed a signature My Goals™ icon to help patients quickly identify tools that would support them in achieving health goals.

Website Development

As a direct counter part to the My Goals™ icon, specific resources on the clinic's website were highlighted with this icon. An additional My Goals nutrition education webpage was built.

Newsletter

Internally I published a newsletter to enable providers to stay up to date with resources being linked to the My Goals icon online.

Tools For Tracking Progress

An application called Fūdrr was developed by Kids Cook to build community around health education. This app was displayed on the My Goals™ prescription pad and online resource page. By using Fūdrr patients can track their progress and quickly share summaries with their physician.

Records and Patient Care Plan

The clinic often scanned and uploaded documents to the electronic medical record system. The My Goals™ prescription pad was designed to complement with this system to ensure it would be fully integrated into a patient's care plan.

Patient Care Connections

The bright colors and slim design of the My Goals™ prescription pad encourages patients to post their goals on their home refrigerator. Additionally, patients who are a part of the Healthy Foods Center program are encouraged to share their My Goals™ prescription with Roadrunner Food Bank's health and wellness team. Patients who have self selected to share this information are able to obtain greater support from nutritionists and dietitians without encoring additional medical expenses.
